Every reading, certification and incident is kept as a dated record of what your team entered. Months freeze when they close, so a closed month isn't re-scored against today's rules.
If history were recomputed whenever the rules changed, last March would quietly re-score itself against a policy written in August. OnDeck freezes each month when it closes, so a closed month's report reflects what was on record at the time.
The MAHC binder is for the health inspector. The insurance binder is for your carrier at renewal. The training report is for the person above you. They are different documents because they answer different questions.
Each submission records the staff member who made it and when. On a shared pool-deck iPad, staff pick themselves before submitting — with an optional PIN if you want one. (Anonymous safety concerns are the deliberate exception.)
